Surrogate Family Care is located in New Hyde Park, New York, and was founded in August 2013 by Premal Jani. A physician by trade, Premal greatly understands the complexity of the medical world and how it impacts an individual’s overall wellbeing. But she’s always enjoyed taking care of people too. For her, creating Surrogate Family Care was a seamless transition — providing the opportunity to offer insightful home care services to those who needed it, while staying involved in direct care, her passion.

What started as companion care services for adults 18 and older, Surrogate Family Care has gradually added more care options — thanks to more licensing from the state of New York (a truly rigorous process, but one that has expanded their service offerings and demonstrates their commitment to care). They are now able to meet the needs of individuals at all different levels of care. Services include, but are not limited to:
- Companionship
- Skilled nursing and therapies
- Nutrition
- Social services
- Home health aides
- Housekeeping
So what makes Surrogate Family Care different? Not only is their approach to client care individualized, but caregivers receive in-depth details on a person’s likes and dislikes, preferences, and behaviors — while being oriented to what signs and symptoms to look for when things may seem “just not right” with the patient. Caregivers are given extensive training on proper techniques and an empathetic approach, along with superior communication.
Because of Jani’s medical background, a patient’s whole being is continually being addressed in long-term service delivery — not just his immediate needs. A patient can receive an ongoing diagnostic approach from team members, all while residing among the comforts of home.
Caregivers build relationships on compassion and patience, and keeping the patient safe remains at the forefront. Home care services are currently licensed in Nassau County, Suffolk County and Queens, while companion services are available throughout the state of New York.
